Abstract

본 연구는 치료 합병증이 발생한 퇴원환자를 대상으로 치료 합병증과 동반 질환 간의 연관성을 분석하여 합병증의 예방과 관리 방안을 모색하는 데 목적이 있다. 2011년부터 2020년까지 질병관리청의 퇴원손상심층조사 자료를 이용하여 Apriori 알고리즘을 이용한 연관분석을 하였다. 분석 결과 전체 치료 합병증 환자에게서는 고혈압, 당뇨, 만성 신장질환, 투석을 위한 의료 접촉 과 혈관 인공삽입장치, 삽입물 및 이식편의 합병증(T82) 간에 강한 연관성이 관찰되었다. 당뇨와 고혈압 환자를 배제한 치료 합병증 환자의 경우에는 만성 신장질환과 혈관 인공삽입장치, 삽입물 및 이식편의 합병증(T82) 간에, 악성 신생물(C77, C16, C50, C78)과 처치 후 합병증(T81) 간에 유의미한 연관성이 나타났다. 만성 신장질환과 치료 합병증 간의 높은 연관성(lift = 4.002)은 만성 신장질환자의 표적 집단 관리를 통해 치료 합병증의 예방이 절실히 필요함을 시사한다. 본 연구는 데이터 기반의 분석을 통해 치료 합병증과 동반 질환의 관계를 확인하여 합병증 예방 대상과 방안을 제시하는 데 의의가 있다.

This study aims to analyze the relationship among treatment complications and comorbidities in discharged patients with treatment complications and to explore strategies for the prevention and management of these complications. Data from the Korea National Hospital Discharge In-depth Injury Survey conducted by the Korea Disease Control and Prevention Agency from 2011 to 2020 were analyzed using the Apriori algorithm for association analysis. The results revealed a strong association among hypertension, diabetes, chronic kidney disease, dialysis status, and complications related to vascular prosthetic devices, implants, and grafts(T82) in all patients with treatment complications. In patients with treatment complications excluding those with diabetes and hypertension, significant associations were observed between chronic kidney disease and complications related to vascular prosthetic devices, implants, and grafts(T82), as well as among malignant neoplasms(C77, C16, C50, C78) and post-procedural complications(T81). The strong association between chronic kidney disease and treatment complications(lift = 4.002) highlights the critical need for targeted management of chronic kidney disease patients to prevent treatment complications. This study contributes to identifying high-risk groups and proposing data-driven prevention strategies through the analysis of the relationship between treatment complications and comorbidities.
